Wedding Forms
Floral and Décor Agreement 2025-2026
This contract is for any florists (professional or amateur) planning on decorating the chapel.
This form is for the couple to fill out and return.
Photography Agreement 2025-2026
This contract is for anyone (professional or nonprofessional) photographing at the chapel.
Videography Agreement 2025-2026
This contract is for anyone (professional or nonprofessional) videotaping at the chapel.
Facility Use Agreement 2025-2026
This contract is for the couple to fill out and return when requesting the use of the chapel for weddings.
Outside Coordinator Agreement 2025-2026
This contract is for anyone coordinating the wedding ceremony (amateur or professional).
This fillable form is for anyone requesting to use the chapel affiliated with TCU students, staff, or faculty.
